List of Documents Required to Attend Orientation
This list consists of the minimum requirements for agency accreditation in our industry.
All documentation listed below must be current and in our office prior to your orientation.
(It is not necessary to have these documents when you apply; we will contact you later regarding these documents).
All information listed below is subject to change.
1.) Two years of caregiver experience. References are required and will be verified.
2.) Current driver’s license or state I.D. card.
3.) Signed Social Security Card (Must present an ORIGINAL signed card at our office. We will copy and return your original).

4.) Current T.B. (tuberculosis) test results. (Must be current and renewed every three years).

5.) Criminal background check from the LA State Police. This can be obtained at a cost of $26 at our office or at the office of the Louisiana State Police in Baton Rouge. Click here for details on the background check. (Must be current and renewed every five years).
OPTIONAL: At this time, the Caregiver Registry Standards Board is not requiring a C.P.R. card except for child care placements. A C.P.R. card is highly recommended. Click here to access the American Heart Association’s website to find C.P.R. classes.
